Eye bag surgery Chiangmai
Eye bag surgery Chiangmai : Eye Bag Surgery / Lower Blepharoplasty) is a cosmetic surgery procedure to correct sagging, puffy, or wrinkled skin under the eyes due to aging. It involves removing excess fat, rearranging its position, or trimming excess skin and muscle to create a smoother and more youthful appearance under the eyes.
Causes of under-eye bags
Genetics: Some individuals have a protruding orbital fat structure from a young age.
Aging Process: The skin and muscles around the eye (Orbicularis Oculi) lose elasticity, and the ligaments under the eye (Orbital Septum) weaken, causing the orbital fat to protrude.
Anatomy: The presence of a deep tear trough makes under-eye bags more visible.
Who is lower eyelid bag surgery not suitable for?
In general, lower eyelid bag removal surgery can be performed on people of all ages and genders, as long as they do not have certain health conditions that may cause side effects. For example,
Diseases or abnormalities of the eye area.

Eye Bag Surgery / Lower Blepharoplasty
Lower blepharoplasty is a procedure to correct problematic lower eyelids. Typically, the lower eyelid is puffy and bulging due to excess fat under the eye, which is held in place by strong eyelid muscles, resulting in a smooth and taut appearance.
However, stress, lack of care, and increasing age can cause this fat to gradually bulge out and become more noticeable. The best way to reduce under-eye fat bags is to surgically remove the excess fat or transfer it to areas with deep wrinkles to create a smoother appearance.
And to correct sagging skin and muscles in the lower eyelid, liposuction under the eyes is not possible because it is close to the vital organ, the eyes. Lower eyelid surgery cannot correct wrinkles, eyelid sagging, and crow’s feet, but it will tighten the muscles around the lower eyelids to make the eyes look younger and prevent inward valgling (eyelashes turning inward). This method corrects problematic lower eyelid features, which usually involve puffiness and increased fat in the area under the eyes.
This fat is usually held in place by strong eyelid muscles, giving the eyelids a smooth, taut appearance. However, stress, lack of care, and aging can cause this fat to gradually bulge and become more noticeable. The best way to reduce under-eye bags is to surgically remove the excess fat or relocate it to deeper grooves or creases to smooth out any looseness and correct sagging skin on the lower eyelid muscles. Liposuction is not a surgical procedure to remove under-eye fat.
Because it is close to the vital organ of the eye, lower eyelid surgery cannot correct wrinkles, eyelid sagging, or crow’s feet. Instead, it tightens the muscles around the lower eyelid to give the eyes a more youthful appearance and prevent inward-turning eyelashes.
Transcutaneous Approach
Method: An incision is made under the eyelid, approximately 1-2 millimeters from the lash line.
Advantages: Allows for the simultaneous removal of excess fat and the reshaping of sagging skin/muscle.
Suitable for: Older individuals or those with significant wrinkles, sagging, or wrinkled skin under the eyes.
Under-eye Fat Repositioning
Method: Instead of simply removing the fat, the doctor will reposition some of it, moving it to fill in deep tear troughs.
Advantages: Solves both tear trough and under-eye bag problems simultaneously, reducing the chance of deep under-eye hollows after surgery. LANNA
Pre-operative Facial Structure Assessment
Snapback Test / Pinch Test: This test assesses the elasticity of the skin and muscles of the lower eyelid by pulling the skin down and releasing it. If it takes longer than 2 seconds to return to its original position, it indicates poor skin elasticity, a warning sign of a high risk of ectropion (downward eyelid roll) if too much skin is removed.
Tear Trough Rating: This assesses the depth of the tear trough. If the tear trough is very deep, the doctor may consider fat repositioning or micro-fat grafting to prevent the under-eye area from appearing sunken.
Negative Scleral Show / Vector Check: This checks the cheekbone alignment relative to the eye. If the cheekbones are flat (negative vector), the lower eyelid will lack natural support, and special care must be taken to avoid scar contracture.
Preparation before lower eyelid and eye bag surgery.
• Blood thinning medications, such as aspirin, should be discontinued for at least 7-10 days.
• You should wash your hair before the surgery, as it may be difficult to wash your hair in the initial period after the surgery.
• On the day of surgery, refrain from wearing makeup or using cosmetics that are difficult to remove.
• You should arrange for someone to care for you after the surgery; you should not drive yourself.
Post-operative care for lower eyelid and eye bag removal.
• First 1-3 days (swelling reduction):
Cold compresses: Apply cold compresses frequently around the eyes and cheekbones.

Avoid straining/bending your head down, lifting heavy objects, coughing forcefully, or rubbing your eyes.
Wound care:
Cleanse with saline solution: Gently clean the wound with a cotton swab soaked in saline solution, then apply medication as prescribed by your doctor.
